Grok | The linked incident is a complete breakdown in IT procedures, which should result in everyone from the shop system administrator to the IT director getting fired. Either there were no procedures, or if there were, the procedures were not being followed, or not being audited for accuracy, not being tested for disaster recovery, or the procedures in place were incompetently specified.
