Could someone please give me the link to the original Lich image shown in that picture above?  It would be very much appreciated, thanks.

Nevermind, found it.  Now my next question is, is would the image from that template be difficult to create?  It doesn't have to be like it (and I don't want it to be flash like the first one is, just like the 2nd/3rd which is just a plain image).

If you are an outstanding 3D modeller then no, not at all. If you want to pay somebody to do it, check out www.graphic-forums.com and gl on anything under 200$.
